EXCLUSION OF LIABILITY ASSUMPTION OF RISK
PARTICIPANT(s) PLEASE READ CAREFULLY

I understand that there is an element of risk of injury in participating in the sport of hockey and/or the
playing of hockey or Pond Hockey or shinny on an ice service created on a natural body of water or
outdoor surface. As a condition of the participant using the facilities and equipment of The Columbia
Valley Rockies Hockey Club, The District of Invermere, the Columbia Valley Chamber of Commerce,
and the BC Ministry of Forests, the undersigned participant assumes all risk of personal injury, death
or property loss or any consequential or indirect loss, resulting from any cause whatsoever, including
but not limited to the inherent risks involved with engaging in the sport of hockey and/or the playing of
hockey, Pond Hockey or shinny on an ice surface created on a natural body of water or outdoor
surface; the use of The Columbia Valley Rockies Hockey Club, the District of Invermere and BC
Ministry of Forests recreation sites and facilities, equipment, lands and premises, or negligence,
breach of contract, breach of statutory duty of care or breach of the Occupiers' Liability Act on the part
of The Columbia Valley Rockies Hockey Club, the District of Invermere, Columbia Valley Chamber of
Commerce and BC Ministry of Forests recreation sites and facilities, their employees, agents,
sponsors, principals, directors, servants, and/or representatives, and/or the owners, lessors or
occupiers of all lands and premises upon which the participant may from time to time be engaging in
the sport of hockey and/or the playing of hockey, Pond Hockey or shinny on an ice surface created on
a natural body of water or outdoor surface.
The undersigned, enters into this release freely and hereby waives and agrees irrevocably not to make
or bring any claim, cause or action, or legal proceeding of any description, which the undersigned may
have, at any time, against The Columbia valley Rockies Hockey Club (including it’s Directors and
Booster Society), the District of Invermere, Columbia Valley Chamber of Commerce, and BC Ministry
of Forests recreation sites, and/or their employees, agents, principals, directors, servants and/or
representatives, or against the owners, lessors or occupiers of all lands and premises upon which the
participant may from time to time engaging in the sport of hockey and/or the playing of hockey, Pond
Hockey or shinny on an ice surface created on a natural body of water or outdoor surface.
