WAIVER, RELEASE AND INDEMNITY

PLEASE TAKE THE TIME TO READ THIS DOCUMENT CAREFULLY BEFORE YOU SIGN IT. IT AFFECTS YOUR FUTURE LEGAL RIGHTS.

IN CONSIDERATION OF THE ORGANIZERS OF THE CAMEL TOE CUP (the "Event") AGREEING TO PERMIT ME TO PARTICIPATE IN THE EVENT, AND FOR OTHER GOOD AND VALUABLE CONSIDERATION, THE RECEIPT OF WHICH IS HEREBY ACKNOWLEDGED, I HEREBY ACKNOWLEDGE AND AGREE AS FOLLOWS:

I acknowledge that I am at least eighteen (18) years old as of August 24th, 2018, or my parent or legal guardian has read this waiver in its entirety and has consented to its terms.
I understand and acknowledge that I assume all risk of personal injury which may result from my participation in the Event. I further understand and acknowledge that I assume full responsibility and risk for my attendance at the Event's venue and the use of any other services or facilities that are open to the public at the venue's premises during the Event, which I acknowledge are independent from and not associated with the Event.
My participation in the Event as a participant or otherwise is completely voluntary. I voluntarily accept all risks of competing in the Event, including my own actions, the actions of others (including but not limited to other participants, Event staff, volunteers and spectators).
I understand and acknowledge that all applicable rules for participation in the Event, and all directions and instructions given by Event staff and volunteers, must be followed and that SOLE RESPONSIBILITY FOR MY PERSONAL SAFETY REMAINS WITH ME AT ALL TIMES while participating in the Event.
I understand and acknowledge that I will be responsible to reimburse the Event, in full within 5 days of notice, for the cost of any property damage for which I am held responsible by the Event staff or officials.
I voluntarily agree to wear the specified equipment per my respective sport as set out in the WRL rules for ringette players & the ASHL rules for hockey players.
I certify that I am physically fit and have no medical condition or health-related reasons or problems that would make my participation in the Event more hazardous, and I have not been advised not to participate by a qualified medical professional.
I undertake and agree to remove myself from participation in the Event should I sense or observe any hazard or unsafe condition, or feel unfit or unable to safely continue.
By signing this waiver, I agree to RELEASE, HOLD HARMLESS AND WAIVE ALL CLAIMS that I have or may have in the future against Camel Toe Cup, its officers, directors, employees, volunteers, insurers, venue, sponsors, contractors and assigns (hereinafter collectively referred to as “the Releasees”) for all loss, damage, injury, death or expense on any kind to me, others, my property or the property of others, as the case may be, resulting or arising in any way, directly or indirectly, from the Event, including without limitation my participation or presence at or traveling to and from the Event, due to any cause whatsoever, including but not limited to negligence, gross negligence or negligent rescue on the part of any of the Releasees and from any breach of contract, statutory duty or duty of care on the part of any of the Releasees.
I agree to have my name, voice, statements, photograph, video, image and/or likeness, and the name, voice, statements, photograph, video, image and/or likeness of my child(ren) and/or ward(s), recorded by the Releasees (collectively the “Recordings and Materials”), to be used for any legitimate purpose by or for the Event, including advertising or promoting future similar events.
I agree not to sue or commence action against any of the Releasees for any act, omission or failure of any of the Releases associated with, related to or in any way connected, directly or indirectly, with the Event or my participation therein, and I further agree to indemnity and hold harmless, the Releasees from all claims, expenses, fees, liability, damage award or cost of any type howsoever arising from my participation in the Event, including any loss or damages resulting from or arising out of their use of the Recordings and Materials.
This Waiver, Release and Indemnity will govern my actions and responsibilities at the Event.
I consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during the Event, which medical treatment will NOT be paid by any of the Releasees.
This Waiver, Release and Indemnity shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
This Waiver, Release and Indemnity shall enure to the benefit of me, and be binding on, my heirs, executors, administrators and personal representatives.

I HAVE CAREFULLY READ AND UNDERSTOOD THIS WAIVER, RELEASE AND INDEMNITY and am aware that by signing this document, I am waiving substantial legal rights on my own behalf, and on behalf of my estate, heirs, executors and next of kin, including giving up the right to sue, and I sign this Waiver, Release and Indemnity of my own free will.
