BEAT THE HEAT 2018 HOCKEY TOURNAMENT, PRESENTED BY HOCKEY COMMUNITY

ACKNOWLEDGEMENT OF RISK, WAIVER AND RELEASE OF LIABILITY

I waive any claims of any nature now and in the future against and discharge from liability Hockey Community, its employees, the organizers of this tournament and Planet Ice Delta should injury, death, loss or damage sustained by the participant occur for the duration of, but not limited to, the tournament from August 10, 2018 to August 12, 2018.
I am fully aware of the collective risks and hazards associated with participating in the physical activity of ice and ball hockey including but not limited to, the following:
1. Intentional and unintentional physical contact and collisions with other participants, boards, nets and other apparatus involved in ice and ball hockey;
2. Sticks, pucks and all other equipment which, whether intentional or unintentional may cause physical injury;
3. While the rules and regulations impose restrictions for play, the participant fully recognizes that he/she may be subject to slashing, hooking, tripping, cross checking, high sticking, spearing, pushing, shoving and similar activities that may result in personal injury.
I acknowledge that I am familiar with the premises and know and consent to the Risks (collective) and acknowledge that the organizers of Beat The Heat 2018 Hockey Tournament have taken steps to reduce risks to the participants. I therefore accept and agree to the terms of this waiver.
I am over the age of 18 years of age.
I agree that any photographs and videos taken at the tournament is property of Hockey Community and its partners.
I understand that the terms of this waiver is binding on the participant, heirs, executors, guardians and personal representatives of the participant.

I have fully read this waiver, understand the terms, and voluntarily sign to allow the unconditional release of all liability to the greatest extent allowed by law.
